Community survey

The Community Survey is a tool implemented by the Community Prevention and Wellness Initiative (CPWI) communities. This survey is typically implemented every other year starting in the summer through the fall however the cadence may vary as needed. Results are used as part of CPWI Community Coalition’s needs assessment as part of their Strategic Planning process.

Survey and other resources - 2026 implementation

This year’s implementation of the Community Survey was delayed due to transitioning to a new survey design using the Service Now platform. The Survey will be implemented from the end of January through the end of July 2026. Given the delays, communities must reach 75% of their target. Unfortunately, this year, HCA is not able to accommodate adding community specific questions.
