"Take Five" Self-assessment Tool for Youth
TAKE Five (https://takefivemi.org) is an online self-assessment tool to support youth substance abuse prevention, mental health and wellbeing. The tool was created by Mercer Island Youth and Family Services (MIYFS) Healthy Youth Coalition in partnership with the University of Washington Center for the Study of Health and Risk Behaviors (CSHRB) with funding from a SAMHSA Strategic Prevention Framework-Partnership For Success grant.
TAKE Five offers any Washington youth (age 13+) the ability to get personalized feedback about the relative risks associated with their personal mental health and substance use status. The feedback includes links to appropriate resources as well as normative information to support positive behavior change that is based on the youth’s responses to questions. Normative feedback is generated from uploaded Washington State Healthy Youth Survey (HYS) data.
TAKE Five has the potential to be modified to offer youth localized resources and referrals as well as have HYS normative data updated over time. The program can be seen as a “youth version” of CSHRB’s Check In With Yourself tool for young adults.
