Healthy Youth Survey: Question revision and new question proposal

The Healthy Youth Survey Planning Committee (HYSPC) is now collecting proposals for new and revised questions to include in the 2025 Healthy Youth Survey (HYS). If you would like to submit a proposal for a new question to be added to the survey, or you would like to propose a revision to an existing question or question scale, please complete and submit this form no later than Friday, April 12, 2024.
